Intraoperative findings and histological investigations of the adito-antral region in patients with some forms of suppurative otitis media have shown, that in 76-80% of cases the blockade of the aditus was evident. Taking this fact into account, we should think that the conservative treatment of these patients is not reasonable, they are assigned to surgical intervention. The choice of technique of surgery depends on the findings during operation. The author performs antro-mastoidotomy, endaurally tympano-autro-adititomy by his own modification and osteoplastic autro-attico-mastoidotomy.

Keywords: cavum tympany, retrotympanic space, aditus ad antrum, tympanic isthmus, tympanic diafragm
